緊張性頭痛（きんちょうせいずつう、tension headache, tension-type headache）、あるいは緊張型頭痛は、頭痛の最も一般的なタイプである。痛みは首・後頭部・眼・体のその他の筋肉に広がっている。緊張性頭痛は頭痛のタイプの約90%を占めている。人口の約3%は慢性的緊張性頭痛を持っている[1]。

詳細は「緊張型頭痛」を参照 緊張型頭痛が発症する原因としては、精神的・身体的ストレスや筋肉の緊張などが複雑に絡み合っていると考えられている。この種の頭痛に関係すると考えられる項目は次の通りである。 身体的ストレス 原因としては無理な姿勢・合わない枕・目の酷使など。特に目や肩などにストレスが集中してかかると周囲の筋肉がこわばって血行が悪くなり（肩こり）、さらに疲労物質などがたまって周囲の神経を刺激し、頭痛を招くと考えられる。パソコンなど、長時間テレビ画面・モニタなどに向かって作業に従事する人などによく見られる。 精神的ストレス 原因としては心配事や不安・悩みを抱えることなど。これによって自律神経がうまく機能しなくなると、筋肉が緊張していなくても頭痛を訴えることがあるとも考えられている。このことは人間の性格にも左右される一面があり、例えば几帳面で律儀な人・生真面目な人ほどこの症状が現れやすいとも言われている。 いずれも女性に多く、数日持続する。緩徐に進行し、典型的には、頭をとりまくはちまき状に痛む。ストレスやうつによって起こり、主に頚部・側頭部の異常な筋収縮に起因する。 治療は、NSAIDs・筋弛緩薬やチエノジアゼピン系、抑うつ症状に三環系抗うつ薬などが使われる。低い枕で寝ることも有効。
