【概要】 核酸系逆転写酵素阻害剤で注目されてきた重篤な副作用。乳酸は骨格筋、脳および赤血球でブドウ糖の代謝経路の最終産物として、嫌気的にピルビン酸から産生される。乳酸の濃度が18mg/dLを超えて増加し、血液のpHが酸性であれば乳酸アシドーシスという。急性脂肪肝を伴う高度の肝臓腫大が起こり死亡率は高い。危険因子として、女性、肥満、HCV/HBV感染、そして長期間の薬剤使用などがあげられている。d4T、ddI、ddCと"d薬"が多いと言われ、最近は選択されにくくなった。
【詳しく】 初発症状は多様。非特異的な胃腸症状のうえに肝酵素の上昇がみられ、呼吸困難がある。集中治療が必要。服薬中止で回復する例もある。原因としては、核酸系逆転写酵素阻害剤がミトコンドリアのDNAポリメラーゼγを阻害してミトコンドリアDNAができないため発生する代謝障害である。
【対策】 薬剤性と考えられたら、すべての薬剤を中止する。治療薬としてカルニチン、ユビキノン、ビタミンB2、ビタミンB6などが提案されているが確立していない。対症療法が中心となる。

乳酸アシドーシス（にゅうさんアシドーシス、英語: Lactic acidosis）は、乳酸（多くはL-乳酸）が体内に蓄積され、血液中のpHが過度に低くなる事を特徴とする病態である。これは代謝性アシドーシスの一種で、身体の酸化的代謝に問題がある為に過剰な酸が蓄積されるものである。
