『イーゼンハイム祭壇画』（イーゼンハイムさいだんが、独: Isenheimer Altar、仏: Retable d'Issenheim、英: Isenheim Altarpiece）は、ドイツ・ルネサンス期の彫刻家ニコラウス・ハーゲナウアーと画家マティアス・グリューネヴァルトが1512-1516年に制作した祭壇画である[1][2]。フランスのコルマールにあるウンターリンデン美術館に展示されている[2][3]。
