ヘパリン起因性血小板減少症（ヘパリンきいんせいけっしょうばんげんしょうしょう、英: heparin-induced thrombocytopenia、略称: HIT）は、抗凝固薬であるヘパリンの投与を原因とする血小板減少症（血小板数の減少）である。血小板はトロンビンを活性化する微粒子を放出するため、HITは血栓症（血管内での血栓の異常形成）の素因となる。血栓症が確認された場合には、その状態はHITT（heparin-induced thrombocytopenia and thrombosis）と呼ばれる。HITは、血小板を活性化する異常な抗体が形成されることによって引き起こされる。ヘパリンを投与されている患者で、血栓症の新規発症や悪化、または血小板数の減少がみられた場合、特異的な血液検査でHITの確認を行うことができる[1]。

詳細は「ヘパリン起因性血小板減少症」を参照 ヘパリン起因性血小板減少症（HIT）の場合、ヘパリンの投与を中止する事が重要である。しかし、それ以降は、一般的に血栓症を避ける為の治療が行われる。治療には、レピルジン（英語版）やアルガトロバンなどの直接トロンビン阻害薬（英語版）が使用される。その他の血液希釈剤として、ビバリルジン（英語版）やフォンダパリヌクスが使用される事もある。出血ではなく血栓症が主な問題であるため、血小板輸血（英語版）はHITの治療には日常的には使用されない。ワルファリンは、血小板が正常化するまでは推奨されない。
