セレブロリシンまたはセレブロリジン(Cerebrolysin)は、ブタの脳から精製されたペプチドの混合物である。脳由来神経栄養因子、グリア細胞株由来神経栄養因子、神経成長因子、毛様体神経栄養因子等から構成されるがこれらに限らない^[1][2]。

脳卒中^[3]や脳血管性認知症の治療に用いられるが、この用途を支持する証拠は弱い^[4]。スマートドラッグとしても販売される。

恐らくアミロイドβの沈着を減少させることによって^[5]、脳血管性認知症患者の認識機能を改善するといういくつかの証拠が得られているが確定的ではない^[4]。ロシア、東ヨーロッパ、中国、他のNIS諸国やアジアの国で広く利用されている^[6]。

リサーチ

これらのレビューでは、さらに質の高い研究が必要であることが強調されている^[7]。さらに、使用は入院を必要とする自然発生的副作用の高い発生率と関連しているかもしれない。アジアの亜集団における虚血性脳卒中の研究では、有益性の欠如が認められている^[8]。

2020年の研究では、脳動脈瘤に伴う出血性脳卒中に有益性は認められなかった^[9]。

初期の研究では、脳損傷^[10][11]、統合失調症^[12]、多発性硬化症、脳性麻痺^[13]、脊髄損傷 など、幅広い神経変性疾患におけるセレブロリジンの使用の可能性が示されている^[14]。

副作用

急性脳卒中後のセレブロリジンの使用を検討した研究では、入院を必要とする「重篤な副作用」のリスク増加は認められなかった。 具体的には次のようなものだ： "投与量を問わず、死亡の原因となった、生命を脅かす、入院を必要とした、または既存の入院を延長する結果となった、永続的または重大な身体障害／労働能力を失う結果となった、先天異常／先天性欠損症であった、または医学的に重大な事象または反応であった"^[15]。

作用メカニズム

In vitroおよび動物実験から、セレブロリジンの神経栄養作用は内因性の神経栄養因子に類似していることが示唆されているが、その特異的な分子薬力学は不明である^[16]。認知症の研究では、βアミロイド沈着の減少が示唆されている。
