硬膜外麻酔(こうまくがいますい、: epidural anesthesiaepidural)とは、脊髄硬膜の外側に麻酔薬を投与することによる区域麻酔の一種である。硬膜外ブロック: epidural block)とも表記され、同義語ではあるが、ペインクリニックでは硬膜外ブロックと表記されることが多い。 医療従事者からはepiduralを略して「エピ」と呼ばれる事もある。
